EHRs typically aren't "made on" the FHIR standard, though they may provide interfaces. If you're really wanting an EHR, the only open source one I'm aware of is OpenMRS. I know they have some degree of FHIR support. I don't know exactly how much. If you're just wanting an open source FHIR server, there are a few available - look at https://wiki.hl7.org/Open_Source_FHIR_implementations
