In the Baltimore HL7 meeting it was my understanding that the -00:00 offset was to be interpreted as UTC time without knowledge of time zone whereas +00:00 was to be interpreted as an offset that happens to be in a GMT time zone. Has that been brought into FHIR yet and if so, where can I reference it?

To my knowledge, that's never been agreed to. It was proposed, but it's not supported by most (any?) libraries and is therefore rather problematic.

It’s an obscure hack to solve an obscure problem in an obtuse standard (v2). We won’t be adopting it in fhir under any circumstances

Yes, it sounds pretty obtuse. (btw, then there's also the UTC with the TZ represented by "Z")
