Seems very odd to record demographic about a patient using Observation. Or am I misunderstanding how this might be used in FHIR?
And I have yet to encounter this in a CDA doc.

It is ABSOLUTELY used - at least the quality measure content is. It is central to quality measure reporting in the USA.
