Which Loinc code is used to represent "Examination" in the Observation.code in scenario 1? What codes are often used for the same Observation pattern for past history/surgical history etc?

Basically this:
Observation.code represents the nature of the observation and the Observation.value a code represents the non-numeric result value. These are two distinct facets that are central to a FHIR Observations. For example:
code=[Examination] (Which code do I use for this?)
value=[Abdomen tender]
