In that case: No, you can have a US-Core profile that constrains the base resource. Based on that you have a "new york" profile maybe, on top of that you can have a "new york laboratory" profile, and then maybe a profile that shows how a certain new york hospital has limited that to their specific situation.
This can go as deep as you would like.

The only limit is your sanity and how many profiles you can load into the memory of the machine doing the validation :)
