FYI @Grahame Grieve it looks like test.fhir.org is either misconfigured or has a bug. The links on search pages are referring to localhost rather than its external name.
See e.g. http://test.fhir.org/r2/Patient/_search?_content=&_content%3Amissing
The "This Resource" link on patient 1 links to http://localhost:92/r2/Patient/1/_history/1
