We have a message distribution that is a centralized distribution between endpoints. So at each endpoint you have doctors using software. They use the software to communication with other doctors using software, all communicating through a centralized messaging system.

The messageheader has source and destination elements. the source and destination elements have endpoint information, which represents the systems that are partaking in the exchange. If you also wanted to record the receiver (person) and there may be multiple people at the same endpoint receiving the message, is it acceptable to have multiple destination elements, each with the same endpoint but different practitioners?
