Topic: MedicationStatement not taken has gone away - why?

In MedicationStatement R3 there was a "taken" element where you could capture in a MedicationStatement that a particular medication was currently being taken - or was not currently being taken. This has been removed in R4. Is that because the intention is to use the status of "not done"? If so, it would be good to call that out in the description/usage notes.

Yes, that is correct. Workflow alignment essentially replaced "taken" with a status of not-taken.
