How/where would 'patient not to be given medication XYZ' be represented in FHIR? (patient wishes, not allergy)
Would 'hospital doctor recommends that GP prescribes drug ABC' be represented as CarePlan in FHIR? (e.g. referenced in a discharge summary Composition)

It is possible that this would fall to the Consent resource (record of patient wishes regarding handling). But that resource today has focused on Privacy-Consent, so you won't find anything specific there (e.g. advanced directives). That workgroup (CBCC) would like to get your input, and involvement.

@Philip Scott There's been a bunch of discussion around how to capture "do not do", but that hasn't necessarily landed yet. For now, it would probably be MedicationOrder with a modifier extension

I'm sure that this is not a Consent issue; The patient may not consent to - say, tranfusions - and than these are contra-indicated

I brought consent up for the specific part of the use-case where the patient expresses a 'wish'... I understand that a patient expressing a 'wish' is something that is assigned to the Consent resource to solve (e.g. DNR). I am not saying that it is a general solution for instructions, surely not about medication dispensing instructions.
