Topic: Mapping of diagnosis free text to Condition elements

Hi, in the application, we have free text fields for Diagnosis “Clinical Description” and “Details”. Would the first one map to Condition.code.text even if it is a free text field that can diverge quite a bit from what the code represents? Or would you use notes for both free text fields? But then how to distinguish them?

@Martin von Siebenthal Can you provide an example? I would like to better understand how far off the clinical description might be from the code.

@Michelle (Moseman) Miller Thanks for your reply. By default we even fill in the code meaning (i.e. same as FHIR coding.display). So without the user doing anything that's a perfect fit. But I have no idea how far the users edit that field, and since they can, we have to assume they do put anything they don't find another field for. But then again in most cases it will pass as "refinement" of what the code expresses. I'll try to collect some clinical examples...
