Hi @Jose Costa Teixeira this is what I'm doing in the IT base guide http://www.hl7.it/fhir/build/it-base/site/StructureDefinition-Paziente-mappings.html. (still under development); considering to have both in the same IG.

If logical models and profiles are maintained by different governance groups; in my opinion, ConceptMap may be a better solution allowing to decouple the two artifacts (Just my point of view).
