Author is who performs a particular operation. The organization that the patient is a "patient of" might be the only organization allowed to update the Patient instance, but that's not a given. (Some organizations might allow external participants, including the patient themselves to update certain demographic information). Also, the author might be captured at a level of detail that is more precise than just the organization. E.g. "what practitioner changed this phone number".

A use case to consider is found in long-term care. A skilled nursing facility might be where the care is occurring, but we could have a wound care specialist or external hospice agency come in to provide complementary care. That might help with understanding how they might be different. If not, apologies...
