From an site implementation perspective, I would expect this would be handled as either a transfer (if the two facilities are closely related and using the same ADT system), or as a leave of absence->admit or discharge->admit (depending on if the source site supports LOA or not).

From a FHIR modeling perspective, I think all of those options are covered in Encounter. You either have one encounter where the location changes (in the case of the transfer), or two encounters, one that goes "onleave", and a second that is created at the target facility.

In the onleave/discharge -> admit scenario, you do have to deal with discontinuing/re-ordering everything (diet/med orders, etc.) of course, but I think that is outside the scope of what you are asking about?
