How do we say "this communication is being sent out to the careteam on behalf of the care plan"?
How to link to the care plan?

"on behalf of the care plan" doesn't make a lot of sense. It can be sent in fulfillment of the care plan, but "on behalf of" has to be an entity capable of making decisions and taking responsibility - which a CarePlan isn't

"in the scope / context of the care plan" is more what I mean.
Example I got was communications that are themselves not fulfilling the care plan.
Things like "the patient has given the care team a box of chocolates to show appreciation for last week's activities.".

we use the careplan as umbrella for activities and communication with or about the patient in the scope of their disease.

Sounds like you should be using EpisodeOfCare. CarePlan is just what you're planning to do. If it's not part of the plan or because of the plan, it's not really related to the plan...
