We need to implement a KOOS (Knee-Injury-And-Osteoarthritis-Outcome-Score) questionnaire. http://www.koos.nu/koos-english.pdf
Has anybody worked on it already and has a model that can share?

For whoever may need this in the future, LOINC.ORG has a KOOS (and other questionnaires) defined.
