Observation.referenceRange.appliesTo has cardinality 0..*
So, within a single referenceRange, it is possible to have one appliesTo element expressing "female" and another one expressing "American Indian".
I couldn't find a place in the spec where it is defined whether this should be interpreted as logical AND or OR.
E.g.: is the above example only relevant for female American Indians, or is it relevant for every Patient who ist either an American Indian or a female or both.
Am I overlooking that information or should there be a clarification?
