the answer is that the iG publisher doesn't make any use of .page. Whatever is in there is passed through untouched to the user (though it is on my list to check that all the things in .page and under actually exist)

I don't have any plans at this stage for the .page content to be generated by the IG. Note that @Lloyd McKenzie has a pre-processor that populates this based of some other content (or is it the other way around?)

what is in .page is key pages, not every page (a decent IG will have 100s (or not 10000s) of pages...

Actually, the IGPublisher does make use of page - it will generate a table of contents and will generate data files allowing the inclusion of section numbers

In respect of adding a sub-page for a 'toc', for the 'source' tag with a value of 'toc.html', does one have to make an empty html page or will the IG Publisher create a page called toc.html?

it should be a fragment that you put in a real page with a jekyll include

The fragment name is toc.xml. You can put it where you like.
