I'm reviewing over the patient and was considering how Aliases are represented and discovered that to do this you just add more name entries into the patients Patient.name property with the use of old.
I do note that this use property is marked as a modifier extension, however none of these values appear to change the meaning of the name.

because one of the values is 'old' - this is not a current name for the person
