In Ontario the HealthCard number is called an OHIP #.

In many discussions i've had, people have talked about using this is a patient.identifier (with some extensions thrown in to capture things like version code).
.
However, would it be more appropriate to be used as a "Coverage" resource connected to the Patient?
Or both? Or something else?

I think both. It's absolutely used as a patient identifier - even in situations where reimbursement isn't going to be sought. However, it may also be appropriate to capture it as a Coverage - especially for out-of-jurisdiction visits
