Goal.Description says "Code or text describing goal" but the code set seems more of a condition set than a goal set.

We could clarify the text to be "Code or text describing goal or condition that is supposed to be addressed"

but that would possibly conflict with the long description of the element:
Human-readable and/or coded description of a specific desired objective of care, such as "control blood pressure" or "negotiate an obstacle course" or "dance with child at wedding"

I understand that this is a broad value set but these seem mostly undesirable conditions, not goals (well, 219006...)

See Dave's comment on J#10623, which influenced the binding to that value set. Examples from that value set that are relevant include:

Or, diet-related Goals using child concepts from SCTID: 449921000124105, Able to control portion size (finding) – US Extension
Or, a Goal to become a non-smoker using SCTID: 8392000, Non-smoker (finding)
This value set would also cover partial achievement of a goal, e.g. SCTID: 134406006, Smoking reduced (finding)

These terms are findings - some of which may actually coincide with goals but the concept is not the same.
For example 6 month pregnancy is a finding, but I don't think it is a "goal" concept

I fear some misuse. What would we put if the goal is "Diabetes management"? (I fear some people would put "diabetes" in there - confusing goals with the condition we want to manage is not good)

In short, while some findings may be goals in some circumstances, I don't think we should use that value set. we should clarify the definition or find a different value set - possibly just a sub-set of the one we have now. But current value set seems misleading

Feel free to log a change request for Patient Care to refine the value set to a smaller list of examples. It is just an example binding, so I don't think we need to list every possible goal -- just give a few examples like I listed earlier (or others if you have better suggested goal codes)

Agree on that - We don't have to put all good examples, but we should remove the bad examples.

I don't think "non-smoker (finding) is a goal. It is still a finding.
