Datatype Annotation is commonly referenced to record notes. And in R4 Annotation.text supports GFM markdown. The spec emphasizez that markdown processing may be optional. In this case--> Is it correct to presume that markdown inline images may not supported? GFM/CommonMark does allow inline images.
I currently have a need in ResearchStudy.note where there is text and an image. I'm not sure if markdown was best to force render html (in this case)

Right. You can't presume all consumers will know how to render the image. The intention was to support systems that won't know how to render such things.
