Reason for change is generally captured in Provenance for most resources. Some resources might capture status reason but episode of care seems an unusual place for that.

I would have thought provenance would be a weird place to expect to find status change reasons. Can you log a tracker item for discussion in PA Peter? (and if you think this should be coded - a suggested example binding?)
