FHIR R4 Encounter Resource does not have instantiatesCanonical nor instantiatesUri elements. Is this intentional or a miss in the FHIR spec? Would an encounter not adhere to a protocol or definition?

I would expect, at minimum, to support the workflow-instantiates extensions. Knowing what part of a protocol a particular visit represents is critical for research and I suspect will be important for some measures too. @Emma Jones, feel free to submit a change request :)
