Our Organisation is adding some extensions.
What is the best place to document these to encourage use and adoption by others?
-In the organization's website or somewhere more open source?
- Or should we raise a change request for these ?

HL7's registry (http://registry.fhir.org) is nearing the end of its beta, so that's probably the best location for maing something widely known. If you don't want to wait for that, you could use http://simplifier.org

If you think your extension is one that should be included as part of the specification, then a change request is the starting point for making that happen - though the URL of the HL7-approved one will be different from yours.
