In our care mgmt system there isn't a difference between Encounter and Document We use them interchangeably, along with "Checklist" and sometimes "Scenario." So determining what data belongs in each of these fields is a bit confusing. For DocumentReference -> Context -> Encounter, it's not super clear to us what is being looked for here.

what is the two or more meanings you are confused by? The context of this DocumentReference has a relationship to this Encounter. (Note that in R5 the .context backbone is flattened, so it is just DocumentReference.encounter.. does that make it more clear to you?
