Hi,
If I were compiling a FHIR document bundle reflecting a patient encounter with a copy of the consult note attached under the document reference resource - where should you add the references to the address/CC'd recipients included in the consult letter? In a message bundle the messageheader has a receiver element but I cannot see any in the composition, neither any element in the DocumentReference for attachment metadata - only strictly an author element.

This functionality would be specific to your transport. Thus you need to identify your transport. In the case of the IHE protocols, the intended recipient is an element of the DocumentManifest. This is the manifest of a communication, like a packing slip in a shipping container.
