I would appreciate if someone can provide an optimal approach for condition / Procedure ( strictly conforming to the USCDI standard). I do realize that translation would be required from ICD-9 to the standard terminology: Should we implement in-house terminology server?

Can you expand on what you mean by "condition / Procedure"? Are you looking for concept maps from ICD to CPT? Something else?

As per the USCDI guidelines: "Procedure = SNOWMED CT, CTP or HCPCS 2" and for condition = SNOWMED CT, how do we convert ICD-9/10 codes to SNOWMED or vise versa ?
