Is there any way to update a Patient Telecom with whether or not they still consent to be contacted by that method? I'm hesitant to just remove the field, since this would be for a specific case, and doesn't necessarily apply to the entire hospital.
Does it make sense to add an extension for this case?

If you want to flag a communication as usable for a particular purpose (or not usable for a particular purpose) that's different from one of the existing use codes, then yes, extension would be a reasonable solution.

and you can record a Provenance of that UPDATE to indicate that it is by direct request of the patient to remove that communicaiton method
