Could an example of a codesystem supplement be added to the core spec that shows how an additional language is supposed to look like? @Grahame Grieve? I've seen this https://www.hl7.org/fhir/codesystem-example-supplement.html but that does not really help my users to understand the concept. I would like to have an example that eg. shows how a set of concepts are translated to danish, french or swahili

You'd have to use a modifier extension. I don't think there's a standard way.

... and what is the process once the supplements are identified and created. For my case, that is somewhat trivial where I don't expand the codesystem in the supplement but 'merely' provides a new designation. Would the flow/intend then be to do ValueSet expansions using eg. the designation as parameter in order to get the values in the wanted language. Is that correctly understood?

That is what we did for our COVID-19 Questionnaire IG - we used a CS supplement to generate language-specific ValueSet expansions with display values in the right language, e.g.

We currently generate our expansions with custom code, so not sure how/if that would work with e.g. HAPI.

Expand is supported by hapi, but i haven't tried it out with the designation parameters
