B (QuestionnaireResponse)
First, Observation is used primarily for capturing data elements that are "true" observations - lab measurements, vital signs, social assessments, etc., while QuestionnaireResponse can be used to capture any types of data, including data that would typically map to other resources (Procedure, Patient, MedicationStatement, etc.).

C (QuestionnaireResponse)
Second, the focus of QuestionnaireResponse includes the specific phrasing and organization of the questions.

I think A suggests that an "instrument" (curated set of questions used to calculate a score) belongs in Observation, and C suggests it belongs in QuestionnaireResponse. B is ambiguous. Can we clarify which resource is better or should be more conventional?

QuestionnaireResponse is useful for data capture/display, Observation is useful for data searching & analysis. Typically I'd expect APGAR or Glasgow to just be captured as Observations. There's no need to have tight control over the phrasing of the questions or the order in which the components are captured. If you did use a QuestionnaireResponse, you'd still want to extract that information into an Observation for subsequent use.
