I have asked before, but have lost the answer... How is a CDA root plus extension to be put into the Identifier datatype?

Basically as in the first example here: https://www.hl7.org/fhir/datatypes-examples.html#Identifier
root => system and extension => value

(or, preferably, the URL synonym for the root OID)

ah, yes... the problem being that conflicts with the recommendation to use <system value="urn:ietf:rfc:3986" />... thus sometimes the system will be rfc3986 (when no CDA root plus extension was used), sometimes the root will be there...

You use that system when you've got a root but no value

Please I added a CR GF#133223 to get what you said as at minimum a narrative statement, or better a normative statement. Having the community guess based on an example is not good enough.
