Does anyone have a sample C-CDA R2.1 with Provenance at the entry level for USCDI data classes such as Care Team and Allergies? Does provenance need to be captured for Reaction Observation within the Allergy section (because it is mentioned as a data element in USCDI)?

note that CDA is not FHIR. CDA can have 'provenance', but does not use the FHIR Provenance.

The entry template id for provenance author participation is not included in any of the sample files published as part of the Cures Test Data package located here...https://github.com/onc-healthit/2015-edition-cures-update-data. Given that and this statement from the Final Rule (below), I am wondering if it is necessary to include the provenance template or if assigned author is sufficient? I just don't understand why there seems to be no samples anywhere except in the IG where the provenance entry template ID is included. Any comments or guidance would be appreciated.
"From our analysis of the C-CDA standard, we conclude that the requirements of “Provenance” data class are already met by the existing C-CDA standard, and will not require any new development."

Just to add to the above question, Template id- 2.16.840.1.113883.10.20.22.5.6 , which is available in CDAR2_IG_CCDA_COMPANION_R2_STU_2019OCT , and CDAR2_IG_CCDA_COMPANION_R2_STU_2019OCT_AppxC, as per which any provenance author should use this template-id but none of the given samples has an author with this template-id.
