Is there a convention for accessing an element inside a resource inside a bundle? http://.../Patient#my-gender for Patient.gender.id = 'my-gender' is pretty clear. What's the http://.../Bundle#??? version (since ids can repeat in the bundle)?

not according to https://tools.ietf.org/html/rfc3986 . you cannot have two fragment identifiers or a # within the fragment identifier
