Hi everyone,
I'm wondering how implementers around the world are handling given names.
Are people using the EN-qualifier extension (value=MID) (https://www.hl7.org/fhir/extension-iso21090-en-qualifier.html) to distinguish when repeating name.given instances are part of the "first" name vs the "middle" name? HumanName datatype defines "Given Names appear in the correct order for presenting the name" re: the order of occurrence.
Have people needed the ability the distinguish the "Middle" name, in order to be compatible with systems that define distinct first-middle-family names? Particularly in cases where individuals' given name may be multiple words, like "Mary Anne".
Interested in hearing thoughts, since I didn't see the EN-qualifier extension mentioned in US-Core. (I see Nictiz uses the extension on https://simplifier.net/NictizSTU3/nl-core-humanname)

I haven't seen many systems using the qualifier. Typically most implementations just take the first as the 'first name' and the remainder as 'middle' names.

it was primarily defined for northern European use but they are re-assessing the use on an ongoing basis
