I'm currently working with the community of french vendors and we have some questions regarding the Appointment resource. Could you tell us how to specify the creator of the appointment and the person who has removed an appointment? Participant.actor (with an appropriate valueSet)? or adding an extension on the Appointment resource?

Creation user wouldn't be an appropriate use of the participants. Just use an extension...
