As part of maintaining the hierarchy based information in US healthcare systems do the structure what ACO follow ACO -> Region-> Organization -> Facility -> Practitioner, even the Payers follow the same? Can this structure hold good Payer->Network/Plan -> PractitionerDirectory/PharmacyDirectory?
