When implementing the Identifier datatype for NCTId for ClinicalTrials.gov entries for use in ResearchStudy and Citation Resources, and seeing Identifier.use has a Required choice from a code set, we did not know if we should use "usual" because the NCTId meets the definition at http://build.fhir.org/codesystem-identifier-use.html of "The identifier recommended for display and use in real-world interactions." and or if we should use "official" because the identifier also meets the definition of "The identifier considered to be most trusted for the identification of this item. Sometimes also known as "primary" and "main". The determination of "official" is subjective and implementation guides often provide additional guidelines for use."
We think we should use "official" and suggest a change to the definition of "usual" to become "The identifier recommended for display and use in real-world interactions, used when such identifier is different from the "official" identifier."
This is related to https://jira.hl7.org/browse/FHIR-32308 which was entered as a question without a proposed disposition but we drafted the proposed change to the "usual" definition.
