I am trying to add a new module in here, but it seems sensitive to where in the file I add it. If I add it at the end I get errors that all the resources are not found. In the middle, it's ok. But that changes the section numbers of other modules, which it seems better to avoid.

This file (same as in the current build) works: hierarchy.xml
This file is identical but has the "Medication Definition" section moved to the end, and shows the issue.
Possibly not what you meant by a diff, but should illustrate it. hierarchy-fails.xml

yes. But why is that a problem? Isn't this creating the hierarchy? Does it need to align with something else? I thought was moving it to a sibling part of the hierarchy. After the last module rather than before it.

you need have the same parent. You're moving it up one in the heirarchy as well as to the end
