GF#10369 - PCWG voted to add 'unknown' to AllergyIntolerance.reaction.certainty (reaction-event-certainty value set). Some use cases are described for explicitly documenting as 'unknown' vs. omitting the element. OK to add this code?

@Grahame Grieve Agree it's relatively late - just checking to see if it's reasonable to add now vs. wait. The GF#10369 discussion gives the rationale for why PCWG and the Netherlands would like to have this explicitly included in the value set.

QI-Core is profiling AllergyIntolerance, but we don't explicitly reference that element, so the change wouldn't affect QI-Core.
