We are adding reasonReference to the Immunization resource to go along with reasonCode. In the Event workflow pattern, there is no invariant that says either a reference or a code can be present but not both. Is there any reason to expect that these two elements should be mutually exclusive in a resource? Or is the idea that both could be populated with either related or unrelated reasons?

Either or both could be present (and possibly many of each)
