GF#18195 proposes the removal of a number of annotation extensions. Helpfully, the task does not enumerate the extensions to be removed. I believe that the following extensions should be removed:

event-note, questionnaireresponse-note, structuredefinition-annotation, codesystem-comment, valueset-comment,

Comments about response Comments captured about the completion of the form in general or about specific groups or questions within it. Allows commentary to be captured during the process of answering a questionnaire (if not already supported by the form design) as well as after the form is completed. Comments are not part of the "data" of the form. If a form prompts for a comment, this should be captured in an answer, not in this element. Formal assessments of the QuestionnareResponse would use [[[Observation]]]

Wouldn't that be done in the profiles that reference the extension?

You can add a CR to stick that text into SDC.
