The FHIR Build Process documentation still has a reference to needing to create a gforge account. Before I update that to say "Github" account, I just want to make certain that there's not some gforge requirement still? @Lloyd McKenzie ?

I also added a bullet under "getting committer access", item d: "Make sure you have an HL7 Confluence and JIRA account, as you'll need to coordinate changes to the build with JIRA trackers"

Confluence & Jira are the same account - so perhaps make that clear?
