in my understanding, the Episode of Care is administrative and static, while care plan captures the dynamic evolution (from planning to completion)

EpisodeOfCare is a collector of stuff - possibility including orders and plans. CarePlan is about "what is to be done" and it allows linking what has actually been done against the plan

CarePlan can collect what has been done - but that requires updating the plan. On the other hand, linking to an episode can be done by anyone

You can in theory, but typically the performer of the activity can't. Instead someone else will make the update later when reviewing the plan - often with the patient.

What we are doing is that the plan coordinator will keep the plan updated, which I think is what you are saying.

The main difference is that when a Procedure or some other event is created, you can tied, whoever is creating the instance can tie it to the EpisodeOfCare immediately. (Though I guess they can also point to the CarePlan using 'basedOn'.) Updating the CarePlan to reflect where the events actually fit into the plan happens later (possibly much later). Also, EpisodeOfCare can group stuff together in the absence of a single over-arching CarePlan

I don't get the part of the careplan being updated later, much later. I'd say it's the other way around - people do their tasks if planned, and the EoC would collect all that

EoC is the grouper for everything related to the episode - plans, orders, events, co-morbidities, etc. Think of it as a 'view' into the patient's record that collects everything of relevance.

In general, the EoC will always be 'current', though it's possible some records might get mixed and get flagged as relevant later. The CarePlan will only change when whoever's maintaining it does so. In some settings, that might be regularly (even daily), however in many cases, it won't happen until the patient comes in for a visit, so it might not get updated for months
