Looking at the spreadsheets, I see that there's a bunch of places where bindings for Coding/CodeableConcept are represented at 'code list' types in the spreadsheet

what this means is that the build will continue to support them - probably until R4 is published. but after that, they will have to convert to codesystem/value set pairs stored in resources.

Take a look in source/observation at codesystem-observation-category.xml and valueset-observation-category.xml

What's the mechanism for adding ConceptMaps if we want to include mappings to SNOMED, v2, v3, etc. when defining value sets. Doing it for CodeLists was easy. Not sure how to do it using the independent ValueSet route at all...

Not off the top of my head. Perhaps just a column on the bindings page that lets you list the URLs of the ConceptMaps?
