As best I can tell, the problem is a slow terminology server. 20 minutes of the build time was taken up doing value set expansions (which at one point timed out). Also, the validation was super slow. One AuditEvent instance took over 2 minutes to validate.
@Rob Hausam, can you see anything wrong? Tx server seems to be 'up'. Not sure how to tell if it is (or has been) running into resource limits or something else that would make it slow.

@Lloyd McKenzie I'm guessing that there may not actually be a tx server problem, but I will take a look and check on that.
