@Grahame Grieve - I suspect the confusion is around BackboneType vs BackboneElement. Looking at the diagrams, it looks like only Timing, Dosage, and ElementDefinition point to BackboneType. All others point to DataType or PrimitiveType. It seems that the differentiator is that BackboneType has a modifierExtension.

What I find a little confusing is that if I go to the pages for Timing, Dosage, or ElementDefinition, there is no mention of or reference to BackboneType. You have to find the JSON definition (which also is not linked on that page) to see BackboneType as the baseDefinition. The Type column in the page's table shows Timing's type as Element -- so if you follow that link up, you see extension and id, but not modifierExtension.

ah yes that's definitely an error on those specific pages. A task to fix that would be good
