I think at one point this tracker was applied: https://gforge.hl7.org/gf/project/fhir/tracker/?action=TrackerItemEdit&tracker_item_id=15108&start=0 I noticed it has reverted. Is it possible to have this reapplied?

As far as I can tell it should be applied. It is the latest build, but not in the 2018dec snapshot.

It should have been applied between September ballot and final.

Wasn't that Dec snapshot just the sept build with a few minor edits. Not the current build, where it is correct

If the Dec2018 build only has the 4 changes, does that mean current build is what will be cut for R4?

Current build is what will drive R4. Dec2018 was just for a very targeted ballot.
