Is it possible to annotate media items e.g. images with concepts (e.g. lesion seen on an image)? Is the tag element (available in meta) appropriate for it?
What about specific annotations on a certain position x,y or in a frame of a video?

I don't believe this has been discussed.
However, you can certainly do this with DICOM, and reference both the underlying image and the annotation from an ImagingStudy. I would hesitate to reproduce in FHIR functionality that is currently adequately handled by other well adopted standards without a very strong use case.
I suggest continuing this discussion in the #implementers stream.
