@Grahame Grieve I did some work a year ago to extract the questionnaire component from this framework and update it to R4, plus fix some issues with value sets for choice responses. I need a few days work to complete the refactoring and updating, planned as a new Swift Package Manager (SPM) package for QuestionnaireKit. PM me if you have examples with specific requirements. I did not include other aspects of the C3-PRO framework, only Questionnaire rendering and QR results. Full credit to the original developer from Boston Children's, of course!!!

Yes, absolutely, as it's primarily based on the C3-PRO framework. So same license, same attribution to Boston Children's author. Just delayed releasing it until I clean up the extract/refactor of questionnaire support.
