Now that R4 is officially published, would it be appropriate for me to submit a pull request to update all of the published hooks to include a list of R4 resources (for the ones that list DSTU2 and STU3 resources)?

Will do. Question - I don't see DeviceRequest in the list of resources for order-review/select/sign proposed hooks in the resource lists for STU3 even though the equivalent - DeviceUseRequest - is listed for DSTU2. I think this may have been an error. Is it appropriate for me to correct it as part of the R4 pull request or should I submit it separately?

And another question - should I update hooks slated for deprecation or should I leave them as is (to help discourage further adoption)

This is a more substantive change than just adding r4 examples. Please make it a separate PR / issue
